Insurance and Payment
Understand your coverage and potential costs before receiving a Welgen One service. Insurance benefits, provider networks and payment responsibilities vary by plan and by the specific care you receive. Our team can help you identify the questions to ask before you schedule.
What to Check Before Your Visit
Coverage for a screening, consultation, test or ongoing program may differ even within the same insurance plan. Confirm these details for the service you are considering.
Your specific service
Ask what is included, which provider or organization will deliver it, and whether separate services may be billed.
Your plan and network
Confirm your plan is active and ask whether the participating provider and location are in network for that service.
Your expected cost
Ask about your deductible, copay, coinsurance, any authorization requirements and available self-pay options.
Please note: A benefits or eligibility check is an estimate based on information available at the time. It does not guarantee coverage, insurer approval or payment.
How to Prepare for Payment
Tell us the service
Contact Welgen One about the screening, program or other service you’re interested in. Please use only an approved secure patient process to provide personal health information.
Review your benefits
Ask which provider will deliver the service and verify the relevant benefits and network status with your insurer using the details provided.
Confirm the next step
Discuss any expected patient responsibility or self-pay price before proceeding. If your care changes, ask whether that may change the cost.
Common Insurance Terms
Your plan documents and insurer determine how these terms apply to your care.
Deductible
The amount you pay for certain covered services before your plan begins paying its share. Some services may be covered before you meet it.
Copay
A set amount your plan may require you to pay for a covered service, such as a visit.
Coinsurance
A percentage of the allowed cost of a covered service that you pay under your plan’s terms.
Out-of-pocket maximum
A plan-year limit on what you pay for covered, in-network services that count toward the limit. Premiums and noncovered services generally do not count.
Your monthly insurance premium keeps your coverage active; it generally does not count toward your deductible.
Does “Preventive” Mean No Cost?
Many health plans cover certain qualifying preventive services without patient cost sharing when the service is provided under the plan’s requirements, often by an in-network provider. That does not mean every screening, lab test or Welgen One service is automatically free.
Coverage can depend on the service, your age or other eligibility criteria, the provider, how the service is billed and whether additional evaluation or testing is performed.
Ask Before You Schedule
- Is this exact service covered under my plan?
- Is the participating provider in network?
- Will a deductible, copay or coinsurance apply?
- Could any follow-up service be billed separately?
For general information, see HealthCare.gov’s preventive services guide.
Self-Pay and Cost Estimates
If you do not have insurance, or you choose not to use it for a service, ask for the self-pay price and what it includes before scheduling. Availability and prices may vary by service and participating provider.
For eligible scheduled care, uninsured and self-pay patients have rights to a good faith estimate of expected charges. Ask the scheduling team how to request one for the service you are considering.
Know What an Estimate Covers
An estimate helps you understand expected charges before care. It is not a promise that every possible service or charge will be needed. Ask who will bill you and whether labs, additional testing or follow-up visits are included.

Frequently Asked Questions
Which insurance plans does Welgen One accept?
Participation can vary by service, location and the provider delivering care. Contact Welgen One with the service you’re interested in, then confirm the participating provider’s network status and your benefits directly with your insurer before your visit.
Will my screening be free because it is preventive?
Not necessarily. Some qualifying preventive services are covered without cost sharing under applicable plan rules, but coverage for a particular screening or additional service must be checked. Ask about your expected cost before participating.
Do I have to meet my deductible first?
It depends on your plan and the service. Some covered preventive benefits may be available before you meet your deductible; other services may be subject to it. Your insurer can explain how your plan applies.
Does a benefits check guarantee insurance will pay?
No. A benefits check reflects information available before care. Final payment depends on the service provided, claim details, plan rules and the insurer’s determination.
What if I receive an insurance statement or bill?
Compare your bill with your insurer’s explanation of benefits, which shows how a claim was processed. An explanation of benefits is not itself a bill. If something does not match what you expected, contact the billing team identified on your statement and your insurer for clarification.
Can I pay without using insurance?
Ask about self-pay availability and the expected charges for your specific service before scheduling. If you are uninsured or choose not to use insurance, you may be eligible to receive a good faith estimate.
