Welgen Global Enterprises, LLC, doing business as Welgen One (“Welgen One,” “we,” “us,” or “our”), is committed to treating patients, program participants, caregivers, and visitors with dignity, respect, and consideration.

This statement describes the rights and responsibilities that may apply when you receive healthcare, screening, wellness, remote monitoring, digital health, or related services from Welgen One.

Some rights and responsibilities may vary depending on the service, location, healthcare professional, insurance plan, employer-sponsored program, and applicable federal or state law.

Your Rights

Respectful and Considerate Care

You have the right to receive care and services in a respectful, professional, and considerate manner.

You have the right to personal dignity, appropriate privacy, and freedom from abuse, neglect, harassment, intimidation, exploitation, and retaliation.

Nondiscrimination

You have the right to receive services without unlawful discrimination based on race, color, national origin, limited English proficiency, primary language, age, disability, sex consistent with applicable law, or another status protected by applicable federal or state law.

Additional information is available in our Nondiscrimination and Language Assistance Notice.

Language Assistance and Accessible Communication

You have the right to request free language assistance services when necessary to provide meaningful access to Welgen One’s covered programs and services.

You also have the right to request appropriate auxiliary aids and services or reasonable modifications when necessary for effective communication or equal access.

Available assistance may include qualified interpreters, sign-language interpreters, translated materials, large-print documents, accessible electronic formats, audio materials, telecommunications relay services, and other appropriate communication methods.

You will not be required to provide or pay for your own interpreter when Welgen One is legally responsible for providing language assistance.

Information About Your Care

You have the right to receive information about your care in language and terminology you can reasonably understand.

Depending on the services provided, this may include information about:

  • Your health status or screening results
  • A proposed evaluation, test, treatment, service, or program
  • The expected purpose and potential benefits of a service
  • Material risks, limitations, and possible side effects
  • Reasonable alternatives when applicable
  • The likely consequences of accepting or declining a recommendation
  • Follow-up care or additional evaluation that may be appropriate
  • The name, role, and professional status of people involved in your care

You have the right to ask questions and receive reasonable responses concerning your care and services.

Participation in Decisions

You have the right to participate in decisions concerning your healthcare and services.

You may discuss available options, express preferences, ask for clarification, and participate in developing a care or service plan when applicable.

You may ask for a second opinion or request referral information, subject to service availability, clinical appropriateness, insurance requirements, and applicable law.

Informed Consent

You have the right to receive information reasonably necessary to make an informed decision before consenting to a healthcare service, procedure, treatment, screening, monitoring program, or other intervention when informed consent is required.

You have the right to ask questions before signing a consent form.

Consent to one service does not automatically constitute consent to every future service or use of your information.

Refusing or Discontinuing Care

You may refuse a recommended service, treatment, test, monitoring program, or procedure to the extent permitted by law.

You have the right to receive information about reasonably foreseeable consequences of refusal or discontinuation.

Welgen One may ask you to document your decision. Refusal of a particular recommendation does not automatically end your relationship with Welgen One, although we may be unable to provide a service safely or appropriately without necessary information, consent, or participation.

Privacy and Confidentiality

You have the right to appropriate privacy during consultations, examinations, screenings, communications, and other healthcare interactions.

You have the right to expect that your health information will be handled in accordance with applicable privacy and security laws.

Additional information about uses and disclosures of protected health information is available in our Notice of Privacy Practices.

Access to Health and Billing Records

You have the right to request access to medical, billing, laboratory, wellness, disease-management, and other health records maintained in a designated record set, subject to limited exceptions permitted by law.

You may request an electronic or paper copy of eligible records. Welgen One may charge a reasonable, cost-based fee when permitted by law.

You may also ask us to correct or amend information that you believe is inaccurate or incomplete. We may deny an amendment request in certain circumstances, but we will explain the denial in writing and describe any additional rights you may have.

Confidential Communications

You may ask us to contact you in a particular way or at a particular location, such as using a specific telephone number or mailing address.

We will accommodate reasonable requests as required by law.

Restrictions on Uses and Disclosures

You may ask Welgen One to limit certain uses or disclosures of your protected health information.

We are generally not required to agree to every requested restriction. However, if you pay in full out of pocket for a healthcare service or item, you may ask us not to disclose information about that service to your health plan for payment or healthcare operations. We will honor that request unless disclosure is required by law.

Personal Representatives and Support Persons

You may designate an individual to participate in discussions or assist you, subject to your authorization, privacy requirements, clinical considerations, and applicable law.

A person legally authorized to act as your personal representative, such as a legal guardian, healthcare agent, or person holding an applicable medical power of attorney, may exercise rights on your behalf within the scope of that authority.

Welgen One may verify a representative’s identity and legal authority before disclosing information or accepting decisions.

Advance Directives

You have the right to provide information about an applicable advance directive, healthcare agent, medical power of attorney, or other legally recognized healthcare instruction.

Welgen One will consider valid advance directives as required by law and within the scope of services we provide.

Because Welgen One may not provide hospital or emergency services, some advance-directive instructions may need to be communicated directly to hospitals, emergency professionals, family members, or other healthcare providers.

Information About Charges and Insurance

You have the right to request available information about charges, billing practices, insurance participation, and expected financial responsibility.

If you are uninsured or do not plan to use your insurance, you may have the right to receive a written Good Faith Estimate of expected charges before scheduled care.

Additional information is available in our Good Faith Estimate and No Surprises Notice.

Cost and coverage information may be estimated and does not guarantee payment by an insurer.

Continuity and Coordination of Care

You have the right to receive information about reasonable follow-up needs, referrals, and continuity-of-care considerations related to the services Welgen One provides.

When permitted and appropriate, Welgen One may coordinate with other healthcare professionals, laboratories, pharmacies, insurers, and service providers involved in your care.

Research, Photography, and Recordings

You have the right to receive appropriate information and provide any consent required by law before participating in research.

You may decline participation in research without losing access to otherwise available care or services, except when the service itself is offered only as part of a research study.

Welgen One will obtain appropriate permission before using identifiable patient photographs, video, audio, testimonials, or case information for marketing or other purposes when authorization is required.

Information About Policies

You have the right to receive information about policies that directly affect your care, participation, conduct, privacy, safety, scheduling, billing, or use of Welgen One services.

Complaints and Grievances

You have the right to ask questions, express concerns, recommend changes, or file a complaint about your care, service, billing, privacy, accessibility, or treatment by Welgen One personnel.

You may file a complaint without being denied medically appropriate services or subjected to retaliation.

Welgen One will make reasonable efforts to review complaints promptly and fairly.

Your Responsibilities

Provide Accurate and Complete Information

You are responsible for providing accurate, current, and complete information relevant to your care or service.

This may include:

  • Symptoms and health concerns
  • Medical and surgical history
  • Medications and supplements
  • Allergies and adverse reactions
  • Prior test or screening results
  • Pregnancy status when clinically relevant
  • Other healthcare professionals involved in your care
  • Insurance and billing information
  • Changes in your health, medications, address, telephone number, or insurance

Incomplete or inaccurate information may affect the safety, appropriateness, timeliness, or cost of services.

Ask Questions

You are responsible for asking questions when you do not understand information, instructions, risks, costs, or your responsibilities.

Tell us if you need an interpreter, accessible communication method, additional explanation, or other reasonable assistance.

Participate in the Agreed Plan

You are responsible for participating in the care or service plan you accept and for following reasonable instructions provided by your healthcare professional.

If you cannot or do not intend to follow a recommendation, tell us so that we can discuss concerns, alternatives, and possible consequences.

You remain responsible for the consequences of knowingly refusing or not following an agreed recommendation, subject to applicable law and the circumstances involved.

Use Medications and Devices as Directed

You are responsible for using prescribed medications, monitoring devices, screening equipment, digital health tools, and other healthcare products according to the instructions provided.

Do not share prescribed medications, assigned devices, account credentials, or personally issued medical equipment with another person unless specifically authorized.

Promptly report concerning symptoms, adverse reactions, inaccurate readings, damaged equipment, lost devices, or technical problems.

Keep Appointments

You are responsible for arriving on time for scheduled appointments and completing required registration or preparation steps.

If you cannot attend, notify Welgen One as early as possible and follow applicable cancellation or rescheduling procedures.

Late cancellations, missed appointments, or late arrivals may be subject to separate policies and fees when permitted by law.

Meet Financial Responsibilities

You are responsible for providing accurate insurance information, understanding your plan benefits, obtaining required referrals or authorizations when assigned to you, and paying legitimate amounts for which you are responsible.

These amounts may include deductibles, copayments, coinsurance, noncovered services, self-pay charges, missed-appointment fees, or balances remaining after insurance processing.

Contact Welgen One promptly if you have a billing question or believe a charge is incorrect.

Treat Others Respectfully

You are responsible for treating Welgen One personnel, healthcare professionals, other patients, program participants, vendors, and visitors with respect.

Threats, violence, harassment, discriminatory conduct, abusive language, sexual misconduct, deliberate disruption, and damage to property are not acceptable.

Welgen One may take reasonable action to protect patients, staff, and visitors, including restricting nonessential access or ending a service relationship when permitted by law and professional obligations.

Protect Privacy and Account Security

You are responsible for protecting your patient-portal credentials, passwords, verification codes, devices, and other account-access information.

Notify Welgen One or the applicable technology provider if you believe your account or personal information has been accessed without authorization.

Respect the privacy of other patients and participants. Do not photograph, record, or disclose another person’s information without appropriate permission.

Use Electronic Communications Appropriately

You are responsible for using the patient portal or another approved secure method when sending sensitive health information.

Do not use ordinary email, text messages, general website forms, or portal messages for medical emergencies.

Electronic messages may not be reviewed immediately and should not be used when an urgent response is required.

Arrange Appropriate Emergency Care

If you believe you are experiencing a medical emergency, call 911 or your local emergency number or go to the nearest emergency department.

Welgen One’s website, general telephone line, email, patient portal, remote monitoring service, and ordinary text communications should not be treated as emergency-response systems unless a specific written agreement expressly states otherwise.

Follow Safety and Facility Rules

You are responsible for following reasonable safety, infection-control, visitor, equipment, and facility rules.

Tell Welgen One about mobility needs, fall risks, communication needs, allergies, or other circumstances that may affect your safety.

Corporate Wellness and Employer-Sponsored Programs

If you participate in a corporate wellness or employer-sponsored program, you are responsible for reviewing the program information, understanding what participation involves, and providing accurate information.

Ask questions if you do not understand what information is collected, what results you will receive, or what information may be provided to an employer, benefit administrator, insurer, or program sponsor.

Participation in a screening does not replace medical evaluation, diagnosis, or follow-up with an appropriate healthcare professional.

Rights of Minors and Personal Representatives

The rights of minors, parents, guardians, healthcare agents, and other personal representatives are governed by applicable federal and state law.

In many circumstances, a parent or legal guardian may exercise rights for a minor. Exceptions may apply when the minor may independently consent to care, when a court or another person controls healthcare decisions, or when another legal limitation applies.

Welgen One may request documentation establishing a person’s authority to act on behalf of another individual.
